As part of the Telecare family, STEPS/STRIDES of Alameda County is an intensive community support service for individuals with severe mental illness, many of whom would otherwise require extended care in institutional settings. Staff provide services to members in a variety of environments -- in a support center, in the member's home, or in the community -- building trust, establishing a partnership, and striving for success. The program serves individuals who are high utilizers of mental health services and who are considered to be at great risk for psychiatric hospitalization.
In this exciting role, you will help clients in their journey towards recovery by
- Providing for the safe and appropriate administration of medical treatment (including medications) in clients homes, hospitals or transitional housing
- Provide documentation of response to treatment
- Communicates clinical changes to the rest of the treatment team
- Actively engaging clients in therapeutic interactions and client education such as medication management.
To be a success, you must have one (1) year of psychiatric experience, an RN or LVN/LPT license and current CPR certification. You should be caring & passionate and want to work in a multi cultural environment as an integral part of a multi disciplinary team. Must be willing to transport clients in your own car. Bilingual is a big plus!
